About Us

 Monica’s Heartbeat operates as the nonprofit arm of Gentleman Home Care, created to serve elders and veterans who are medically stable yet face homelessness, unsafe living conditions, or the absence of a support system upon discharge from hospitals, skilled nursing facilities, or therapy programs.

Through nonprofit funding, public partnerships, and community support, Monica’s Heartbeat provides the same high-quality residential and in-home care services offered to private-pay residents of Gentleman Home Care. The distinguishing factor is that residents served through Monica’s Heartbeat have their Room, Board, Wellness Oversight (RBWO) covered through federal, state, local, and charitable funding sources.

Rooted in faith and compassion, Monica’s Heartbeat exists to ensure continuity of care, preserve dignity, and restore stability for vulnerable elders and veterans during critical transitions.

Residential Services

 Residents served through Monica’s Heartbeat receive the same level of care, oversight, and accommodations as private-pay residents of Gentleman Home Care. Services include, but are not limited to:


  • Safe, furnished residential placement or in-home care
     
  • 24-hour supervision and caregiver support (as applicable)
     
  • Assistance with activities of daily living (ADLs)
     
  • Medication reminders and wellness monitoring
     
  • Nutritionally balanced meals and dietary support
     
  • Transportation coordination for medical appointments
     
  • Personal care and hygiene assistance
     
  • Light housekeeping and laundry services
     


Transitional & Support Services

  • Hospital and rehabilitation discharge coordination  
  • Care plan collaboration with healthcare providers  
  • Veterans-specific support and advocacy (when applicable)  
  • Ongoing wellness oversight and case coordination  
  • Faith-informed emotional and spiritual support (optional and non-coercive)
